Single Enrolment - Terms and Conditions

  1. Membership of the scheme is open to all residents of the Single Enrolment Borough.
  2. Membership of the scheme is free.
  3. Only one Smart Card will be issued per individual.
  4. The minimum age limit of an applicant is TBD, as at September 2007. Applicants under 16 will need their application countersigned by parent or guardian.
  5. The card will need to be produced whenever the cardholder wishes to take advantage of the services offered by the pilot scheme.
  6. The card is only for the use of the cardholder and is not transferable.
  7. The card can only be used according to the terms and conditions laid down by the individual services offered by the scheme.
  8. The card is the responsibility of the cardholder. Lost, stolen or damaged cards will be replaced at a charge of £5.
  9. Single Enrolment Borough Council reserve the right to withdraw/amend the card. The card is the property of Single Enrolment Borough Council.
  10. Any changes to personal details - name, address, entitlement, must be notified to the Smart Card Help Desk: 01443 864042.
  11. All personal data collected and used to produce the card is in accordance with the Data Protection Act 1998 and will not be made available to third parties without your permission, unless in the interest of the detection of fraud or the detection of other illegal purposes.
